Looks good.
Under 'Safe Delivery of Meds to Pts' - in the Information (What) - I would suggest adding 'Concurrent lab result checks.'
We have found that between the time that the Coumadin order is placed, until it is administered, the PT/INR may be have changed. This is especially true when an ongoing daily Coumadin order is placed. The administering nurse should have the most current lab value presented to her/him at the time of administration.

Good point Don. We actually have a process in place where the nursing staff always has to document the last INR with respect to administration of the Warfarin. But it was after this document was posted. Will make the necessary changes.
Also, this type of intervention is even more useful when you have bar code medication administration.
